How does it work?

Ergometrine maleate belongs to a group of medicines called the ergot alkaloids. The way ergometrine works is not fully understood. It is thought to act on two types of receptors, 5HT and alpha receptors. These receptors are found in the walls of blood vessels and in the uterus (womb). When the receptors are stimulated they cause constriction of the blood vessels and contraction of the uterus. Intense contraction of the muscles of the uterus causes the blood supply of the placenta (placental bed) to shrink back into the uterus. It also causes shortening and kinking of the blood vessels in the uterus. Contraction of the muscle in the blood vessel walls causes the blood vessels to narrow and reduces blood flow. Both these actions reduce blood loss following delivery. In normal labour, these contractions occur naturally once the child is born, to prevent bleeding from the placental blood source. If however, this does not occur naturally, ergotmetrine is given. By acting on 5HT and alpha receptors and causing the contractions described, it prevents bleeding after childbirth.

What is it used for?

- Blood loss after child birth (postpartum haemorrhage)

Warning!

- Avoid use in pre-eclampsia (high blood pressure, protein in the urine and fluid retention in pregnancy).
- This medicine may cause drowsiness. If affected do not drive or operate machinery.
- Prolonged use may lead to gangrene.

Use with caution in

- Life long inherited blood diseases which can cause a variety of symptoms, including mental health problems (porphyrias)
- More than one baby in the uterus (multiple pregnancy)

Not to be used in

- Abdominal pain and bleeding during preganancy
- Blood poisoning (toxaemia) in pregnancy
- Children
- Decreased kidney function
- Decreased liver function
- Decreased lung function
- Disease involving the blood vessels
- Elderly people
- First and second stages of labour
- Heart disease
- High blood pressure
- Induction of labour
- Infection of the blood or body tissues with pus-forming or other pathogenic organisms (sepsis)

This medicine should not be used if you are allergic to one or any of its ingredients. Please inform your doctor or pharmacist if you have previously experienced such an allergy. If you feel you have experienced an allergic reaction, stop using this medicine and inform your doctor or pharmacist immediately.

Pregnancy and Breastfeeding

Certain medicines should not be used during pregnancy or breastfeeding. However, other medicines may be safely used in pregnancy or breastfeeding providing the benefits to the mother outweigh the risks to the unborn baby. Always inform your doctor if you are pregnant or planning a pregnancy, before using any medicine.

- The use of this medicine is entirely restricted to the third stage of labour. It is otherwise not recommended for use in pregnancy. Seek medical advice from your doctor.
- The use of this medicine during breastfeeding is not recommended. Seek medical advice from your doctor.

Side effects

Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with this medicine. Because a side effect is stated here, it does not mean that all people using this medicine will experience that or any side effect.

- Headache
- Abdominal pain
- Confusion
- Low blood pressure (hypotension)
- Awareness of your heartbeat (palpitations)
- Dizziness
- Nausea and vomiting
- Sensation of ringing, or other noise in the ears (tinnitus)
- Chest pain (angina)
- Slow heart rate (bradycardia)
- Difficulty in breathing (dyspnoea)
- Narrowing of the blood vessels in the extremities of the body (peripheral vasoconstriction)
- Chest pain
- Inadequate blood flow leading to low levels of oxygen in the tissues of the extremities (peripheral ischaemia), which may lead to gangrene
- Transient increase in blood pressure
