The loss of discs containing personal details of 25 million people was 'entirely avoidable', the Poynter report says.
While individual officials have not been blamed, the report highlights serious structural failings at the HM Revenue and Customs offices.
Chancellor, Alistair Darling, has told MPs that action was being taken to improve data security and that the public was entitled to expect government departments would protect their personal details. He also stated that there was no evidence of any fraudulent activity as a result of the recent child benefit disc loss.
